By submitting and signing this form, I authorize Star Brite Car Wash to store and charge my updated payment method and to reactivate my Wash Club membership at the wash level selected above. Membership automatically renews monthly and will be billed on my anniversary date. The applicable monthly membership fee, and any additional vehicles added or modified as part of this reactivation, will be charged at current rates plus applicable Pennsylvania sales tax. Current additional vehicle rates: Shimmering $12, Shining $17, Shooting $20 per vehicle/mo. If I chose to remove any vehicles, that will be processed when my account is updated. Star Brite Car Wash reserves the right to cancel any membership at any time for misuse or abuse.
